Prescription Renewal

BEFORE YOU SUBMIT PAYMENTS, IT IS YOUR RESPONSIBILITY TO CALL THE OFFICE AND CONFIRM THAT YOU ARE ELIGIBLE FOR A PRESCRIPTION RENEWAL. 
 
*Prescriptions are only provided to ongoing patients of the clinic, who have follow-up scheduled.  We can only renew prescriptions which we originally prescribed.  
 
Upon submission of information through an online portal, the patient understands:
1) Despite efforts to protect the security and confidentiality of online communication, it cannot be entirely secured.  
2) Online services may have a legal right to inspect and keep electronic communication that passes through their system.
